Ayushman Bharat: Transforming Healthcare in India

Ayushman Bharat is a flagship initiative of the Government of India, launched in line with the recommendations of the National Health Policy 2017. The scheme is designed to advance the vision of Universal Health Coverage (UHC) and to achieve the Sustainable Development Goals (SDGs), with a guiding principle of “leaving no one behind.”

The scheme represents a paradigm shift in India’s approach to healthcare, moving away from a fragmented, sectoral model toward a comprehensive, need-based system. Ayushman Bharat seeks to strengthen the healthcare ecosystem through path-breaking interventions that address prevention, promotion, and ambulatory care, covering primary, secondary, and tertiary healthcare services.

The program adopts a continuum of care approach and comprises two inter-related components:

Health and Wellness Centres (HWCs) – These centres focus on providing comprehensive primary healthcare, emphasizing preventive and promotive services.

Pradhan Mantri Jan Arogya Yojana (PM-JAY) – This component provides financial protection and access to quality secondary and tertiary healthcare services for vulnerable populations.

The Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) was officially launched on 23rd September 2018 in Ranchi, Jharkhand, by the Hon’ble Prime Minister, Shri Narendra Modi. PM-JAY is recognized as the largest health assurance scheme globally, offering health coverage of ₹5,00,000 per family per year. The scheme targets over 10.74 crore poor and vulnerable families, benefiting approximately 50 crore individuals, constituting the bottom 40% of India’s population.

Beneficiaries are identified using the deprivation and occupational criteria outlined in the Socio-Economic Caste Census 2011 (SECC 2011), applicable to both rural and urban households. PM-JAY is fully funded by the Government of India, with implementation costs shared between the Central and State Governments. Through this initiative, India is taking a significant step toward ensuring equitable access to quality healthcare for all its citizens.

Benefits

The Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) provides a comprehensive, cashless health cover of up to ₹5,00,000 per eligible family per year. This coverage applies to a wide range of secondary and tertiary care treatments, ensuring that beneficiaries can access necessary healthcare services without facing financial hardship.

The scheme encompasses all expenses related to the following aspects of medical care:

  • Medical examination, consultation, and treatment
  • Pre-hospitalization care
  • Medicines and other medical consumables
  • Non-intensive and intensive care services
  • Diagnostic and laboratory investigations
  • Medical implantation services, if required
  • Accommodation during hospitalization
  • Food services provided to the patient
  • Management of any complications arising during treatment
  • Post-hospitalization follow-up care for up to 15 days

The coverage of ₹5,00,000 is provided on a family floater basis, meaning it can be utilized by one or more members of a family as needed. The scheme does not impose any restrictions on family size or the age of family members.

A significant feature of AB PM-JAY is that pre-existing conditions are covered from day one. This ensures that any eligible individual suffering from a medical condition prior to enrollment can immediately access treatment for that condition under the scheme. By providing such extensive coverage, AB PM-JAY guarantees financial protection and access to quality healthcare for vulnerable families across India.

Eligibility

The Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) has clearly defined eligibility criteria to ensure that the scheme reaches the most vulnerable and underserved sections of society. These criteria differ for rural and urban beneficiaries, taking into account local socio-economic conditions and occupational vulnerabilities.

Rural Beneficiaries:

For rural households, eligibility is determined based on seven deprivation criteria. The scheme covers all families that meet at least one of the following six deprivation conditions, along with automatic inclusion for households that are destitute, living on alms, belong to primitive tribal groups, are legally released bonded laborers, or engaged in similar marginalized conditions:

  • Households living in a single room with kucha walls and a kucha roof
  • Households with no adult member between the ages of 16 and 59
  • Households with no adult male member between the ages of 16 and 59
  • Households with a disabled member and no able-bodied adult
  • Scheduled Caste (SC) or Scheduled Tribe (ST) households
  • Landless households whose primary income is derived from manual or casual labor

Urban Beneficiaries:

In urban areas, eligibility is primarily based on occupation. The scheme includes households of workers engaged in 11 specific occupational categories, recognizing their vulnerability and informal employment status. These categories are:

  • Ragpickers and beggars
  • Domestic workers
  • Street vendors, cobblers, hawkers, and other service providers working on streets
  • Construction workers, plumbers, masons, laborers, painters, welders, security guards, coolies, and other head-load workers
  • Sweepers, sanitation workers, and gardeners (Malis)
  • Home-based workers, artisans, handicrafts workers, and tailors
  • Transport workers including drivers, conductors, helpers, cart pullers, and rickshaw pullers
  • Shop workers, assistants, peons in small establishments, helpers, delivery assistants, attendants, and waiters
  • Electricians, mechanics, assemblers, and repair workers
  • Washermen and chowkidars

By carefully defining these criteria, AB PM-JAY ensures that financial protection and access to quality healthcare are provided to those who need it the most, covering both rural deprivation and urban occupational vulnerabilities.

Exclusions

While Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) aims to provide comprehensive health coverage to the most vulnerable sections of society, certain categories of individuals and households are excluded from the scheme to ensure that resources are targeted toward those in genuine need.

The exclusions include:

1. Individuals who own motorized vehicles such as two-wheelers, three-wheelers, four-wheelers, or motorized fishing boats.

2. Households possessing mechanized farming equipment.

3. Farmers holding Kisan cards with a credit limit of ₹50,000 or more.

4. Individuals employed by the government in any capacity.

5. Persons working in government-managed non-agricultural enterprises.

6. Individuals earning a monthly income exceeding ₹10,000.

7. Households owning luxury items such as refrigerators or landline connections.

8. Families living in well-constructed, solidly built houses.

9. Individuals or households owning five acres or more of agricultural land.

These exclusions are designed to prioritize the benefits of AB PM-JAY for economically weaker and socially disadvantaged families, ensuring that the scheme effectively reaches those who are most in need of financial protection and access to quality healthcare.

Application Process

The process of enrolling for the Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) is designed to be simple and transparent, ensuring that eligible beneficiaries can easily obtain health coverage for themselves and their families. The process is facilitated by trained operators, known as Arogya Mitras, who assist beneficiaries at hospitals or Community Service Centres (CSCs).

To obtain a PM-JAY e-card, potential beneficiaries need to follow these steps:

Step 1: Submission of Beneficiary Details

Potential beneficiaries provide identification details such as the PM letter, RSBY URN, ration card number, or mobile number. Using this information, the Arogya Mitra searches the available list of beneficiaries to determine eligibility.

Step 2: Search in the BIS Database

The Arogya Mitra then searches the beneficiary in the Beneficiary Identification System (BIS), which includes databases such as SECC, RSBY, state health schemes, and additional data collection drives.

Step 3: Individual Identification

Once a potential beneficiary is found in the database, their identity is verified using government-issued documents like Aadhaar, ration card, or other valid family IDs. The scanned copies of these documents are uploaded to the system to validate the individual.

Step 4: Family Identification

The Arogya Mitra then identifies all family members eligible under the scheme using the ration card and supporting documents. These records are uploaded and submitted to the relevant trust or insurance company for approval.

Step 5: Approval or Rejection

The submitted beneficiary and family records are reviewed by the health insurance company or trust. In cases recommended for rejection, a final verification is conducted by the State Health Agency (SHA) to ensure accuracy.

Step 6: Issuance of E-Card

Upon approval by the SHA, insurance company, or trust, a PM-JAY e-card is issued to the beneficiary. This e-card allows cashless access to secondary and tertiary healthcare services at empanelled hospitals.

Through this structured application process, AB PM-JAY ensures that eligible families can seamlessly access financial protection and quality healthcare without unnecessary delays
